Proof of Eligibility Includes; Driver’s License, State ID, Employer Issued work ID, or Pay Stub showing place of employment in Kendall.

*Second Doses - KCHD ensures there will be sufficient vaccine appointments for all those in need of a second dose within the 42 day window.
◊ Second doses for Moderna vaccine can be scheduled between 28 and 42 days after your first dose.
◊ Second doses for Pfizer vaccine can be scheduled between 21 and 42 days after your first dose.

Reminder: Please bring your COVID-19 vaccine record card with you to your scheduled appointment.
*Please ensure that you are registering for the correct dose - first or second, Moderna or Pfizer.
If you register for the wrong dose, no vaccine will be available for you - Please cancel your appointment.
