Proof of eligibility will be required upon check-in. Proof of eligibility Includes: birth certificate, state ID, passport.
Signed attestation form and a parental consent form will be required upon check-in. Please print, complete, and bring the necessary forms to your appointment. (If you are unable to print forms, they will be provided at the clinic for you.) See below.

*Pediatric Second Doses - KCHD ensures there will be sufficient vaccine appointments for all those in need of a second dose within the 42 day window.
◊ Second doses for Pfizer vaccine can be scheduled between 21 and 42 days after your first dose
