Proof of eligibility will be required upon check-in.
Proof of eligibility Includes: driver’s license, state ID, employer-issued work ID, or pay stub showing place of employment in Kendall County.

*Second Doses - KCHD ensures there will be sufficient vaccine appointments for all those in need of a second dose within the 42 day window.
◊ Second doses for Moderna vaccine can be scheduled between 28 and 42 days after your first dose
Second doses for Pfizer vaccine can be scheduled between 21 and 42 days after your first dose
