Mental Health Advisory Board Meetings are Open Meetings (open to the public)

Kendall County Health Department is committed to providing persons with disabilities with reasonable and appropriate accommodations consistent with the requirements of the Americans with Disabilities Act (ADA) and other state and federal laws.  
 
The Health Department will give primary consideration to the requests of individuals with disabilities. Primary consideration means that the Health Department will honor the choice of the individual, unless another equally effective accommodation is available, or that the requested accommodation would result in a fundamental alteration of health department activities or undue financial and administrative burdens. 
 

Requesting Accommodations 

Please use the Accommodation Request Form below to make your request. You should submit your request to the Human Resource Coordinator. If possible, the request should be submitted at least 10 days before the date of the scheduled appointment, service, or meeting.  
 
The Kendall County Health Department will respond in writing or in a format accessible to you, within seven (7) days from the date the request was received.  
 
If you do not receive communication within seven (7) days, please contact the Kendall County Health Department at KCHDHR@kendallcountyil.gov or by phone at 630-553-9100.   
 
 
Note: You are not required to use the request form. You may also submit a request in any form, written, verbal, or otherwise.
