The FILE of LIFE program supports Police and Fire Department personnel by giving them quick access to your vital medical information during emergencies—especially when you or a family member may be unable to communicate due to a medical condition or injury.

Residents with medical concerns, seniors, and any interested community members can receive a FREE FILE of LIFE packet. Each packet includes a door decal, a File of Life Information Card, and a magnetized vinyl holder designed to be placed on your refrigerator.

The door decal notifies first responders that your FILE of LIFE is located on the front of your refrigerator, making it easy for them to find the information they need right away.

A red and white "File of Life" form with fields for name, address, doctor, preferred hospital, and emergency contacts, along with instructions to keep information up to date and review it every six months.

 The information card includes: 
  • medication taken
  • allergies
  • medical conditions
  • blood type
  • recent surgery
  • emergency contact information

This card is then inserted inside the vinyl cardholder and placed on the refrigerator door. 

Everyone who has a medical condition should complete a FILE of LIFE.
