Immunization Information

 

Immunization Payment

Vaccine prices vary depending on pharmaceutical rates. Price estimates will be provided when your appointment is scheduled. Please, no personal checks. We accept cash and/or VISA, MasterCard, Discover, and American Express. A 2% service fee will be added for credit cards. We do not accept insurance for adult vaccines/testing.

Immunization Records

For us to provide you with your documentation request, please follow the steps below:

  1. Click here to download our Immunization Record Request Form.
  2. Mail the completed and signed form to:
    • Kendall County Health Department 
      Immunization Request 
      811 W. John St. 
      Yorkville, IL. 60560  OR
    •  Fax the completed and signed form to: 
      Attn: Immunization Request 
      (630) 553-9604 
  3. A staff member will contact you when the records are ready for pick up.
  4. If you are unable to pick up the immunization records in person, please call the Immunization Coordinator at (630) 553-9100. 

*You must be a parent or legal guardian in order to obtain a copy of a child's immunization record.

Child Immunizations

Vaccines are only available to children who are eligible for the Vaccines for Children (VFC) program and:

  • are enrolled in Medicaid
  • do not have insurance
  • are underinsured

Children must be accompanied by a parent or legal guardian. Please bring in the child's insurance card along with the your (parent/legal guardian) license or state ID.

Back to School Clinics are available on a yearly basis. Check back soon for the schedule!

Child Clinic Hours

  • 1st and 3rd Tuesday from 4:00 pm to 6:00 pm by appointment only
  • every 2nd Tuesday from 1:30 pm to 3:00 pm by appointment only
  • schedule changes may occur

Travel Vaccine Services

The World Health Organization (WHO) advises travelers to consult with a travel clinic four to six weeks before departure if the travel destination is one where exposure to any vaccine-preventable diseases may occur.
