Caregiver Support
Caregiver Support services provide guidance, education, and emotional support to individuals caring for aging loved ones. The program helps caregivers navigate resources, manage stress, and access tools that make caregiving more sustainable and less overwhelming.
SHIP (Senior Health Insurance Program)
SHIP offers free, unbiased assistance to help seniors understand their Medicare options. Trained counselors guide individuals through coverage choices, enrollment, and benefits, ensuring older adults can make informed decisions about their health insurance.
Transition & Eldercare Services
Transition & Eldercare Services assist seniors and families as they move between care settings, such as from hospital to home, or explore long‑term care options. The program focuses on safety, continuity of care, and connecting individuals with the supports they need to age comfortably and confidently.
