Immunization Records

Thank you for requesting immunization records.

In order for us to serve you with your request, please follow the steps below.

Note: You must be a parent or legal guardian to get a copy of a child's immunization record.

Steps to request records:

1. Download Immunization record request form.
2. Mail completed and signed form to:

   Kendall County Health Department
   Immunization Request
   811 W. John St.
   Yorkville, IL. 60560

   OR Fax completed and signed form to:
   Attn: Immunization Request
   (630) 553-9604

3. A staff member will contact you when the records are ready to be picked up.
4. If you are unable to pick up the immunization records in person, please contact the immunization coordinator at (630) 553-9100.
