Mental Health Records

Thank you for requesting mental health records.

In order for us to serve you with your request, please follow the steps below.

Note: You must be the patient, parent or legal guardian to get a copy of the mental health records.

Steps to request records:

1. Download the Mental Health record request form.
2. Mail completed and signed form to:

   Kendall County Health Department
   Mental Health Record Request
   811 W. John St.
   Yorkville, IL. 60560

   OR Fax completed and signed form to:
   Attn: Mental Health Record Request
   (630) 553-9604

3. A staff member will contact you when the records are ready to be picked up.
4. If you are unable to pick up the mental health records in person, please contact the Mental Health secretary at (630) 553-9100.
