Temporary Food Permit Application
If you have not previously obtained a temporary permit from the Kendall County Health Department, if your menu has changed, or if you are a Bake Sale fundraiser or Cottage Food Operator please contact us at 630-553-8026 to speak with one of our staff or email us at [email protected].
In order to provide vendors and applicants convenient access to our application process, you may use any one of the following methods to apply for a temporary food establishment permit:
DESKTOP USERS CAN CHOOSE FROM THE FOLLOWING:
- Fill out the PDF application, save a copy to your computer and email it to the following email address: [email protected]. Click here to pay online.
If using Internet Explorer, fill out the PDF application and email it using the email button in the toolbar in the top left corner above the document. Choose “send copy” to send a copy of the filled PDF to: [email protected]. A copy will not be saved to your computer. Click here to pay online.
If using a browser other than IE, you will need download, save, and send a copy to the above email address.
- Print out the PDF application and mail it in with a check or money order.
FOR SMARTPHONE USERS:
- Ensure the correct app is installed on your phone. You will need an application like Adobe Fill & Sign, TinyPDF, PDF Max 5, or Fill and Sign PDF Forms.
- Follow the directions with the application to sign and send/save the filled PDF.
- Email the completed PDF document to: [email protected].
- Click here to pay online.
Credit card processing fees will be charged for all credit card transactions. Refund processing for incorrect payments may take approximately 90-120 days. Please be aware that your application is complete only after you have submitted the form and paid the correct fee. Incomplete applications will not be processed.
If you are operating out of a commissary kitchen, please submit the most recent inspection of the commissary kitchen and your commissary agreement to us via email [email protected] or fax 630-553-9603.
Environmental health staff will contact you prior to permit issuance. Submittal of payment and/or application is not approval to operate.
