Dental Expense Assistance Program
The Dental Expense Assistance Program is intended to assist eligible clients with the expense of oral health care treatment. This one-time assistance of up to $350.00 per year, not to include orthodontic work, and is paid directly to your oral health care provider.
To be eligible, you must be 18 or older and a resident of Kendall or Grundy Counties; with a household income at or below 125% of the current federal poverty level.
Required Documentation:
- Driver's License
- Proof of residence
- Social Security Cards for all members of your household
- Income information for 90 days
- Estimate from Dental Office
- Statement of Need
