Community Health Improvement Plan
Kendall County Health Department is excited to present the 2021-2026 Community Health Improvement Plan. This plan is intended to positively impact the health and well-being of our community as well as to learn about ways to develop greater efficacy in our implementation endeavors along the way. By carefully assessing and planning for community health needs, we are able to develop a compilation of data-driven information that describes the community health status, and we will be able to discourage redundancies in the public health system, as well as identify gaps in the fulfillment of health and well-being needs. Through a carefully planned action cycle, we will be able to improve performance, enhance public health partnerships, and provide increased assurance with regard to the health and well-being needs of the community. The three priorities determined by community consensus are:
- Promote Opportunities to Reduce Vaccination Hesitancy
- Increase Community Awareness to Protect Against Vector Borne Disease
- Connect Our Most Vulnerable to Assets That Reduce Socio-Economic Duress & Support Mental Health.
